I think that best combination is:

2. Install CCCP ( Combined Community Codec Pack).
When installing CCCP, select ONLY
- Haali Media Splitter
- VSFilter
In CCCP install configuration, select auto-load for VSFilter
3. Configure Mediaportal to use CoreAVC as H.264 codec
4 You're ready to go!

Maybe because coreAVC is payware while ffdshow is free and open source? Besides, it can handle most audiocodecs too whereas with you're combination as far as I can tell you also need a seperate ac3 decoder and so on (assuming the mkv comes with an ac3 track).
