That's correct, they aren't available FTA at the moment but Five and possibly FiveUS and Fiver will be available in the future.

This can be done using 'TV groups' in the TV Server config. Not sure if TV Server defaults to having a group called 'All Channels' or whether I created that myself, long time since I set channels up!
If there isn't one there, create it and add all of your channels to it. Rearranging the channels in this group should then rearrange them in the guide within MediaPortal. You can create multiple groups (for example by genre: Movies, News, Sport etc.) and change between them within MediaPortal but that's taking things a step further.
