I used to run the latest svn (I compiled every other day or so). Since this problem, I've removed the svn versions and went legit by installing the 0.2.0 from the installer. Same problem.

Any ideas of what I can try? I've tried reinstalling directx9, but the dx9 installer didn't do anything (because I have the latest installed). I haven't changed anything (that I can remember).

I had a heap of issues that all seemed to be due to the direct input mappings. In the end it was resolved by using windows update to make sure I had all the latest dotnet updates.
It was strange the way it started happening for (seemingly) no reason, but that's what fixed it for me.

Around the same time there was an update in the download area about a new direct input handler and the old one being just for some legacy issues so I guess that's the connection.

It's similar to one of the things I saw before to installing the July 10th stable patch. I had two pop-ups during initializing DirectX, related to D3D and telling me to install the DirectX Software Developers Kit. After that the start-up seemed to stop, even though a MP process was still running. After a reboot, MP would star up with no issues.
So I installed the stable patch and didn't see this particular issue anymore, but I still have no proper tv playback. See my other posts under "display freezes during tv playback".
