Errors: Unable to create/start graph & No Video/Audio detected

Having some strange and frustrating problems with getting the TV Server to work. I have been beating my head on the computer for two days now and all I can get is "Unable to create/start graph" when I try to start timeshifting a channel. Then when I try to timeshift again, I get "No Video/Audio detected." Is anyone else experiencing this issue? I am attaching some logs that may or may not be useful. Anyone that can help point me in the right direction has my sincere gratitude.
