If updating to latest IMDB+ version did not allow your Custom rename for the titles, then you will need to delete the IMDB+ database from Moving Pictures then try installing IMDB+ again. Did you test?

What do you mean by imdb+ database? Do you mean movingpictures.db3?

Does the Custom titles work after the update? If not remove the imdb+ database and uninstall IMDB+ and reinstall latest.

Updated through Extensions Manager to version 2.1.0.539. IMDb+ is not used at all. I have used an instance of Mediaportal where this is the only update, so no manual changes in Moving Pictures...
