640x480 playback fails (1 Viewer)

I have a problem with most Youtube videos.

When selecting 640x480 (default) playback starts but video doens't show, all I see is a little black box in the top left corner. It stays there after playback is finished
The funny thing is that all other resolutions work perfectly.

Is this a bug or am I having some weird problem?

I have tried Nvidia 190.62 drivers and latest drivers but both don't work.
Changing the screen resolution form 1280x720 to 1980x1080 doesn't work.
I've updated to RC5 update but the problem is the same.

My skin is StreamedMP

I'm running SAF 5

Tried with SAF 4 without result.

Did a fresh install of everything, problem exist now with all .flv resolutions (black box in top corner isn't always showing, basically you could say that I have only audio playback)

Some more info for you guys:

Windows 7 Home Premium 64bit
Nvidia drivers 191.07 (like recommended in SAF thread)
SAF 5 build of today.

Media Portal 1.1. RC5 standard skin (Blue3wide)
Onlinevideos 0.22 (tried install with and without FLV splitter, same result)
nothing else installed

I've now also tried the Cyberlink FLV Splitter with the same result.
My guess is has something to do with FFDShow or Mediaportal failing to put the FLV stream on top.

here's an example where I'm experiencing the problem:
YouTube - Kanal von IGNentertainment

I've added IGN as a custom item in OnlineVideo's Youtube configuration with:
Uploads by ignentertainment

When I click a video, the usual popup shows up
320x240 playback works
640x480 gives audio only
1280x720 works but that's mp4, not flv

This problem is not limited to IGN but appears to be limited to youtube.

By other words, if you download this video to disk and play it localy - does it works?

I'm not sure how to download it.

I use downloadhelper in Firefox and downloaded all files related to the video, however most are in mp4 format, only one is in flv format and is the 320x240 clip

all downloaded files play fine in windows mediaplayer. I don't have a standalone flv player
