Its possible you selected a series or season...and 'Marked All as watched'. Was this the case?
If you download episode information for the whole series, it also fetches episodes in the future...if you then marked all as watched for a given series then you may also be marking future episodes as well. But it will only mark watched the episodes that in your database.
It probably should be updated so it only marks all as watched/unwatched for the items that are currently visible in the facade.
