Problem with setting up TV server with MySQL (1 Viewer)

I forgot to include the problem that I am having
I get to “setup database connection” I select “MySQL5”
I hit “test” and get “connection succeeded” then I hit the save button
From what I see in the installation guide, I should be seeing a TV server configuration screen but it never appears.
When I click on “TV service not running Start now” I click “yes” and then get a
”Setup TV has encountered a problem and needs to close.”
What am I missing ?
