Begin recording other channel while watching TV gives double audio and wrong window (1 Viewer)

If I schedule a program to record on channel A, and watch channel B (both on STB through same card), the following occurs:
Channel B is displayed properly, until it is time to record channel A, at that time:
- the screen goes black for a second or two
- the STB is blasted correctly to channel A
- the My TV screen is displayed with channel A in the preview window
- the "no tuners available" dialog window pops up
- a second later, live channel A is displayed in a large window left of center (see attached photo) and the preview window remains static
- a second audio stream starts at this point, slightly delayed behind the main audio, causing an echo
- I hit Alt-F4 at this point to shut down but nothing happens
- I then press the green button on the MCE remote and am dropped back to the desktop with 1 audio stream still playing (can see two ffdshow audio icons in the systray but 1 immediately disappears)
- the audio stream terminates about 15 seconds later

I've tried this three times on different channels, all with the same result. Let me know if you need additional info, this would be a superb product for me if I can get the TV component stable for me (understand it may be for others).

One additional point - just checked and it turns out the channel A program is recording correctly until the point the program is terminated.
