I cancelled a scheduled recording. Specifically this one:
Fútbol Mexicano on channel 46.1 (Unimas)
It was scheduled to record at Saturday at 5:00pm (pacific time). I cancelled it about 10? minutes before it started.
Even though I cancelled it, Mediaportal went ahead and recorded it anyways.

What I see in the MP log is:
    [2013-09-21 16:56:29,056] [Log ] [MPMain ] [DEBUG] - TVProgammInfo_OnRecordProgram - programm = Fútbol(ID:4291486) on 111 9/21/2013 5:00:00 PM - 9/21/2013 7:00:00 PM
    [2013-09-21 16:56:29,163] [Log ] [MPMain ] [DEBUG] - TVProgammInfo.CancelProgram - programm = Fútbol(ID:4291486) on 111 9/21/2013 5:00:00 PM - 9/21/2013 7:00:00 PM
    [2013-09-21 16:56:29,164] [Log ] [MPMain ] [DEBUG] - - schedule = Fútbol on 146 6/4/2013 6:30:00 PM - 6/4/2013 8:30:00 PM ID=2148
    [2013-09-21 16:56:29,164] [Log ] [MPMain ] [DEBUG] - ProgramID = 4291486 ScheduleID = 2148

So indeed it looks like you may have attempted to cancel the schedule.
On the server side there is no indication that the schedule was removed. PowerScheduler continues to report that that the next scheduled recording is at 5pm:
[2013-09-21 16:57:57,769] [Log ] [PS StandbyWakeup] [DEBUG] - PS: Inspecting Scheduled Recordings: 9/21/2013 5:00:00 PM

Did you have any indication from MP that the recording had been successfully cancelled?
This was a once-off recording, right?

Hmmm, just tried to reproduce and no good.
It looks like you were shown a dialog at the point when you attempted to cancel the recording. Do you remember what the dialog said and which option you selected?

The recording I cancelled is a Series recording. If I remember correctly I selected the upcoming recording from the TV Guide. Went into the program information and clicked on Don't Record. It looked like I successfully cancelled it since the Don't Record button now said Record. The weird thing is that once I went back into the TV Guide the recording still had the red Series Recording Icon. I went back into the program's information to make sure the recording was still cancelled and it still showed Record, as if it was cancelled.

It happened again. I cancelled a recording at around 7:36pm.
Fútbol on channel 8.4 VMAS at 8pm.

I cancelled the recording from the client. The scheduled recording was cancelled successfully.
I checked the TV Guide and there was no recording icon. I finished watching a recording a little after 8pm and noticed in the TV Guide that the cancelled show was recording.

I see this all the time. After a clean install it is okay for a while, and then it starts to happen. Unfortunately it is not easily predictable, so I can't produce logs/database before and after cancelling the schedule so that you can tell whether the schedule is cancelled in the database.

However, I can confirm that when I cancel a schedule from a series, it never shows up again in the 'schedules' screen.

Well I decided to do a fresh install using MediaPortal 1.4. To my suprise the bug is still present.

TV Guide screen. Scheduled recording is cancelled but the weird thing is that the "Series Recording" icon is still present.

Program Info Screen. Here you can see that the program is not scheduled to record:

But even though the recording was not scheduled the recording starts at 3pm

I attached a couple of log files. First one is before the recording starts. The second one is after the recording has started.

When you did a clean install, did you delete the TV database and start from scratch ?

For me, this only starts to happen after a while.

We are doing major work on the TV engine at the moment, so please don't expect a quick fix.

If this happens on a clean install with a fresh database, then you are not seeing the problem I am seeing, which would be useful to know.

I first uninstalled 1.5 and when the MySql dialog popped up I clicked delete.

I then did a clean install of 1.4 then upgraded to 1.5 (to see if it was just an issue with my old install).
But the issue was still present.

I then used the BackupSettings plugin to backup my settings. I uninstalled 1.5 clicking on delete once again when MySql dialog popped up again.
I then installed 1.4 and used BackupSettings plugin to reinstall.
