I checked the upscaling setting in Madvr and found if I used anything other than NED13, settings as shown in the 2nd screenshot, the problem did not occur. I seem to recall from the release notes of an earlier version of Madvr that there is some restriction with NED13 but can no longer find it, so maybe that is the cause.
