The log says: "FindFilterByName returned '0x80040216'", although it does not say which filter I am pretty sure the MPUrlSourcefilter is not found. Can you check with GraphEdit if it is registered, if not manually register the ax ? It is in the MP Installation folder (subfolder MPUrlsSourcefilter).
