I found it.

It looks like the executable filename's extension-part has to be lowercase.

After I changed the filename from MK4.EXE to MK4.exe, I could add the game.

I have reproduced this with many other applications as well.

Nevertheless, could someone else please cross-check this so we could file a bug report?
