DVB-T channel scan bug
When a channel changes it's name or is replaced by a new channel that uses the same transportId & serviceId, the old channel is left in the database.

The channel scan logic should UPDATE the existing channel or DELETE the older channel.

To reproduce you'll need to manually edit your TV database to simulate a channel name change and then rescan.
