Thanks for the log files (y) There are certainly continuity errors (discontinuities) showing in both the IPTV source and TsWriter log files. Continuity errors mean chunks of the stream are missing. In general they're most commonly caused by:

Inadequate signal strength/quality.
Overloaded time-shifting HDD (too full, or highly fragmented, or heavily used [often by security software scanning the time-shift files]).
...and in your case, network related issues could also play a part since all of your tuners are network-attached tuners. For example, you might have a bad/overloaded network link between your PRIME and/or HDHR and the PC on which TV Server is installed, or maybe you have a driver or configuration issue with your NIC and/or network security software. There are other possible causes, but those are the most common. The log files can't tell me what the specific cause or cause(s) are. Further investigation must be done to figure out which cause(s) apply for you. However, if I had to guess - based on the frequency of the errors (every 2 to 5 seconds) and the fact that the same continuity errors are present in the IPTV source log as in the TsWriter log - I would guess your problem (at least with streams from the PRIME) is inadequate signal strength/quality, a network-related issue, or something related to the CableCARD or PRIME. Is the problem only happening with cable streams (ie. the PRIME), or only with over-the-air streams (ie. the standard HDHR), or both? What are the signal strength/level (unit = dBmV) and signal-to-noise ratio (SNR) values shown in the PRIME's webpage when you're observing the problem?
