Yes, All 8 are required. They describe all possible mediafiles source FPS and what it should do.

I have some different than yours.:
Name > Framrate > Refreshrate
Cinema > 23.976 > 23 (with an ATI card this is picked up as 23.976hz and is different than 24 and my TV supports this.)
Cinema24 > 24 > 24

NTSC > 59.94 > 59 (This is actually 59.94hz)
HTSCHD > 60 > 60
HTSCFILM > 29.97 > 59 (My TV supports this, because it is actually 59.94hz.)

that looks perfect, covers all the fps, and the refresh rates are all correct.
some fancy TVs / video cards might both support 23.976 so if both support this you could use this as the refresh rate, which would be even better than 24hz

I would think that
23.976 -> 24hz is superior to 23.976 -> 23hz

With an ATI "23" is picked up as 23.976hz. But I think "23.976" as refreshrate is now working as well.

I've never seen a GPU setting that would allow anything but whole numbers
for both ATi & Nvidia:
23Hz will do 23.976
24Hz will do 24Hz
59Hz wil do 59.94

Yeah, all true. It is a MP thing. I changed the post you quoted, because I mixed things up in my mind (MP setting/actual hz)
