Please look at the *.doc - it indicate the file in my folder, saved as a dtd file. Is this correct ?
Yes, that seems to be correct. (y)

In the setup for MC2XML :

Note: you may also create a batch file to auto rename the xmltv.xml file to tvguide.xml for you.

Can someone tell me how to make a batch file to covered up the above ?
    The tutorial explains it here:
    http://wiki.team-mediaportal.com/1_...)/XMLTV_mc2xml#PowerScheduler_Plugin_Settings

Just create a text (.TXT) file with the described content.
After the file has been created, change the extension from .TXT to .BAT in the same way as you did for .TXT to .DTD.

I found the : c:\ProgramData\Team Mediaportal\MediaPortal TV Server and below this I found the catalogs : Log - Recording - timeshiftbuffer - shall I just make the folder xmltv below the MediaPortal TV Server ?

The location of the XMLTV folder does not matter. You can put it wherever you like and call it whatever you like.
The only requirement is that you must put your XMLTV file (tvguide.xml) and xmltv.dtd in a single folder.
Then when you configure the XMLTV plugin, you tell the plugin where you put the tvguide.xml file.

In the middle of December this year The ZapTV will start up to cover Norway, so hopefully i will get the data for input MP from there.

...so you can pay for the donator version of mc2xml (in order to use the "Microsoft" service), or swap to a different XMLTV provider (eg. WebGrab+ etc.)

ok but how can I pay the donator version ? As I see it I can use the schedules direct but this is just for N/S America, and the other ones looks like I can use in Norway but I have no idea how to pay for the username/password.

As it says on the mc2xml website and in your screenshots: Paypal!

You donate to receive a version where the "microsoft" option is not disabled.
