I have seen quite a few posts about missing episode thumbnails and I was in the same boat. I found out what my issue was and it might help others as well!
On the Import Settings Tab go to the Online Data Sync section and there is a check box for "Download Episode Thumbnails" that is not on by default. I checked that box and bingo it worked!
So everyone who is having issues, make sure that box is checked. Now all I need is to get the icons working and im finally done.
