Could anyone help me, plese, with multiswitch commands?

I have SkyLink Sky 0504 4x4 multiswitch, which uses 13/18V & 0kHz/22kHz commands to switch b/w 4 inputs.

Could anyone tell me what to use in DiseqC section to match the following:

LNB A: 13v/0kHz
LNB B: 18v/0kHz
LNB C: 13v/22kHz
LNB D: 18v/22kHz

I have TechnoTrend TT-1401 TV-card with latest BDA drivers, and all these run under Windows 7 Beta 7000.
