Look at this small video. First the file embedded album thumb is shown then only the two artist thumbs are shown and no album cover anymore.

This makes sense.
However FH doesn't work as designed in this case. Although MediaPortal provides the album thumb (MP properties) FH doesn't show it. Only the FH artist thumbs are shown. See this video and this log file.

FH work as Disigned, if no Album art found, FH try found Artist art. Its by design.

I see several cases where the album MBID is correctly resolved and actually MusicBrainz has a front album cover on CoverArtArchive but FH does not download it:
