What version of MediaPortal do you have installed? Are you using an SVN build? I looks like the DLL does not match your installed version...
Yes, I use the latest SVN build.
Also looking at the instruction, you said not to extend the desktop into the 2nd monitor, I'm not sure how to do that since disabling extended desktop also disable the 2nd monitor (unless that's how it's supposed to be for the plugin to work).
I got it to work now with the 0.2.3 verson (non SVN) using plug in release 12/9/07 version.
Thanks for your hard work, now, for the 1st time, the touchscreen becomes useful.

A new version of the driver is available... See the first post in this thread.
Important note: This driver's design has changed significantly. It is no longer included in the "ExternalDisplay" framework and further updates will only be in the seperate plugin. If you have used the previous driver, you must remove it before enabling the new plugin.
The plugin is still based on the ExternalDisplay plugin by JoeDalton, but the ExternalDisplay framework is incorporated into a seperate DLL (named TouchScreenDisplay.dll). This allows the driver to be updated without risk of breaking the mediaportal installation or interfering with (or breaking) other process plugins.

If no one else report some similar problems i will do a reinstall of xp and rebuild my system..

1) Your primary display device (monitor/TV/projector) should by configured as your PRIMARY display device using your display adapters configuration utility. Your primary display resolution can be anything that is supported by your display device.
2) Your Touchscreen LCD should be configured as your SECONDARY display using your display adapters configuration utility.
