Where does the TV server get its episode info from?

I try to set a name of %title% - S%series% E%episode% - %name%

and for some shows I get
Title - S01 E101 - Name

instead of the wanted
Title - S01 E01 - Name

Some shows are putting the season info into the episode info, and I cannot figure out why.
I'm using Schedules Direct for EPG..

Where does the TV server get its episode info from?
In general: from whatever source you're using to acquire your EPG data (program information).
In specific: since you mentioned SchedulesDirect, I'd assume that would be your EPG data source, and therefore that would be the specific answer for you.

So to be clear, the problem is the episode 101?
If yes, I don't see the problem. Who's to say that there aren't more than 99 episodes in the season?

Some shows are putting the season info into the episode info, and I cannot figure out why.
Unless I've misunderstood, it seems like you're assuming that there are a maximum of 99 episodes in a season, and that if the episode number is 101, that must actually be a season number. If that that's the case, with the greatest respect, I would doubt your assumption unless you can provide a concrete example with external proof (eg. an IMDB or TVDB link to the correct episode info) of the problem.
