I have seen that the MPInstaller utility listed the BlueTwo wide skin twice, therefore I have created a different MPI. I have also removed BlueTwo being the default skin in this MPI. Maybe this one works for you.
I have been trying to reproduce your problem on my installation, but couldn't let it happen. Perhaps the problem is that tvgemist uses the common skin file common.facade.xml. When the Monochrome skin has placed the facade on the left side instead of the right, this explains the problem you are experiencing. The only solution in that case is to create custom skin files for the Monochrome skin. Anybody up for doing this?
For me, it's in C:\ProgramData\Team MediaPortal\MediaPortal (ProgramData is a hidden directory)

When using the BlueTwo Wide skin in MP everything works and looks just fine. I'm gonna try to see where Vista is getting it's image previews from, hopefully that really is the root cause of the problem for me.
