Fix your TV codec settings (MP Config -> codecs and renderer section -> TV codecs tab). They're currently unset, which can cause many many problems.
My advice is never use MP without configuring codec settings.
P.S. Don't be confused. When you go to check, you will see that LAV is selected... but that is just the MP Config default. MP does not have default codecs, and so "random" codecs are used. As your log file shows, MP was using "MPEG Audio Decoder":

That codec is notorious for causing problems like this.
If anyone ever experiences the above, I found a fix. Removing and reinstalling MP-TVSeries fixed it. It must have gotten corrupted during the reinstall of MP. I also had to do the same with TRAKT which had some issues too that were not apparent at the time. All is well once again. :cool:
