Can you please disable the Upmixing from Stereo -> 5.1?
Then it will work.

The reason is that i have logioc to detect when the new stream has a different sample rate or channels and then i need to recreate the mixer, which causes the hard stop.
This should not happen in case of upmixiong.

you are right...without upmixing fade in and fade out works properly like it should! Do you need additional log files without enabled upmixing?

Ok i made a full MP cleanup on my desktop pc (for testing) and installed your package from the first post but its not working still the same problem..

ok here we go...inside the logs there are many errors because my htpc with the tv server wasnt available....thats normal

I installed the newest build from scythes rework/fixes for MP 1.4.0 and with this build, the fade in/out is working correctly...will make some more tests to make sure i am right
EDIT: Sorry i was wrong...i forgot that i enabled the upmixing feature of my sound card driver for testing...
