Don't use blue two wide at all, so i'm not sure...Is it supposed to look more like the attached image? If it is, then replace the dialogTvGuide.xml with the attached text file (drop the extra .txt extension, done so i could post it to the thread).
My guess is that changes in the default skins are made initially to blue two, copied into bluetwo wide then converted to widescreen format..and a couple of screens didn't get converted (like the tvserver home screen). All i've been doing is taking sections from other parts of the skin that are scaled correctly, and pasting them into the file that need them.
With all the screens that now make up a skin, its easy to overlook one here and there.
Off topic a bit, Nemulate I notice from your screenshot that you seem to have quite a few tv channel logos. Is there any chance you could post them somewhere?

MP(1) will only use LAV if you select "Internal DirectShow player" as music output, but then you lose gapless playback.
