In recorded TV, if you have a folder containing 7 episodes (for example) and scroll down the list, you get 1/8 to 7/8 showing.

FFDShow DXVA video decoder can cause a crash-to-desktop when removed during graph rebuilding on live TV channel changes. It looks like it needs a full pin disconnect and maybe also a check to make sure the graph is stopped before RemoveFilter is called.
