That's only the case when MP2 is not activated. As soon as you activate the MP2 application (click on it or switch to it with Alt-Tab), it is set to always on top.
That behavior is by design to avoid MP2 being on top of other applications when it is not active.

The problem is that the behavior is currently not consistent:
normal windowed mode -> ALT+ENTER -> MP2 fullscreen, taskbar hides automatically
normal windowed mode -> maximize window -> ALT+ENTER -> MP2 fullscreen, taskbar stays on top, click on MP2 won't hide taskbar

I don't think so, because MP has no way of knowing at which timestamp the external player stopped playback.
Best way to achieve this would be to use an auto-resume feature in your external player itself. (MPC-HC can do that, I don't know about VLC)

Certainly the vintage MP2 installer that I was using no longer installs the LAV filters. I can only assume that these were never part of the install package itself, but rather downloaded during the install, and that link no longer exists. Shame that the installer doesn't indicate this.

I don't know about MP2 but as you said you tried MP1 too, how is the MP music config ?
MP(1) will only use LAV if you select "Internal DirectShow player" as music output, but then you lose gapless playback.
If you you don't have multichannel music you can choose WASAPI as the output and set the number of speakers to stereo.
