One thing i have done in FCU fix with V450 to V451 is that when the trick is executed, MP is getting focus, so i'm not sure if it's really needed so maybe worth a try to test without that part of code and see if all is ok :p

Attached is a bin to go this way, so if it works then maybe this 'focus' part of code is not needed after all :)

With no focus still short flickering before start of video, no flickering after stop. No black screen issues.

    Thanks @Chisum
I did notice in your logs that one part of madVR code was trigger with EVR and it should not.
Maybe it can explain the flickering on video start :)
Does the flickering appears before the refresh rate kick in or after ?

So it can also explain this little flickering too lol

Here with lav. Not sure, if better, still flickering. After choosing the video screen gets black for a short moment, then MP flickers, then short black again and then video plays.

Thanks @Chisum i fear that i will be not able to fix the flickering on start :(
So not sure what to do next, i didn't see flickering if the GPU is not removed, so i think that when Intel will fix their driver, it will be OK :p
Are you able to test with madVR mode to see how it goes ? (to compare)
    :p

Here's my log with your latest build. Still working fine for me for refresh rate change and resume. I can't tell any difference from the V6
