i know there seems to be no one solution for this issue but.... i have now fixed 2 machine that had the same issue as mentioned here.
I updated to the latest MP version at the time 1.1 + SVN that i download 10th august plus the latest beta version of SAF this resolved the issues for both machines. I also just updated them yesterday...

If you don't need the menus (just the chapter points) then MakeMKV will rip DVDs to .mkv files with chapters in one step, and 'mkvmerge GUI' - part of MKVtoolnix - will re-mux existing video files to .mkv
