It looks like the executable filename's extension-part has to be lowercase.
After I changed the filename from MK4.EXE to MK4.exe, I could add the game.
I have reproduced this with many other applications as well.

It surprises me that there hasn't been a plugin that has been specifically designed for this, but we make do with what we have...
I had to reinstall 3.1.1 of my emulators and then copy over the files to get the unofficial myemulators to work. Also, I got rid of my old database.
