I have the same issue as you, with Yamaha rx-v765 and LG PK760 TV. HDMI out from an i7 3770k PC.
When i set the Audio out to TV or TV+AMP i got only Dolby Digital, but when i set it to AMP i got Dolby Digital, DTS Audio and Dolby Digital Plus.
Only if i turn the TV off then it shows TrueHD and DTS-MA.
I can't solve this problem.

You can also try LAV set to DXVA (copy back) as this is supposed to allow connection to post processing. Will increase cpu usage as data is 'is copied back' from GPU memory to CPU memory. ATI being more cpu intensive so than NV in this respect.

Problem solved - muppet network setup. I had configured a static IP address on the client PC with my router as the DNS server rather than my actual server. As I'm running in a domain, the server then assumed that the client was outside the domain, and blocked some (but not all) ports.
